    “套餐”还是“单点”？基层医疗机构诊查费到底应该怎么收？

    　　公立基层医疗机构是基本医疗服务体系的重要组成部分，包含社区卫生服务中心、乡镇卫生院、村卫生室等，其诊疗量占全部诊疗量比例一直保持在50%以上，保障了广大居民能在15分钟内就近获得医疗服务。一些网友反映，到基层医疗机构挂号缴费就诊时常有困惑，同样是基层医疗机构，为什么有的收取一般诊疗费，有的则收取诊查费？两者之间到底有什么区别？ 　　什么是一般诊疗费？ 　　2010年，国务院办公厅印发《关于建立健全基层医疗卫生机构补偿机制的意见》(国办发〔2010〕62号)，明确将挂号费、诊查费、注射费(含静脉输液费，不含药品费)以及药事服务成本整合为一般诊疗费，即不再单独收取上述费用。通俗来讲，一般诊疗费就是将基层医疗卫生机构日常提供的医疗服务进行打包，实行“套餐式收费”，仅限社区卫生服务中心、乡镇卫生院、村卫生室等公立基层医疗机构收取，主要考虑是基层医疗机构受限于技术能力，大量手术治疗类项目无法开展，将基层常见医疗服务进行打包收费，有利于稳定基层医疗机构收入。 　　各地一般诊疗费的价格水平如何确定？ 　　医疗服务价格实行属地化管理，由省级医保局和部分具有价格管理权限的地市级人民政府制定具体价格水平。《国务院办公厅关于巩固完善基本药物制度和基层运行新机制的意见》(国办发〔2013〕14号)提出，各地结合实际合理确定基层医疗卫生机构一般诊疗费标准，原则上10元左右。各地按此标准结合实际进一步明确了一般诊疗费收费标准，但价格水平存在一定差异。2024年，国家医保局编制印发综合诊查类立项指南，保留“一般诊疗费”价格项目，并指导各地加快对接落实。截至目前，已有24个省份对接落实综合诊查类立项指南，从价格水平看，各地价格水平基本稳定在10元左右。对于一般诊疗费，各地医保部门均纳入医保基金报销范围，报销后患者只需支付1-3元。 　　一般诊疗费或门诊诊查费怎么收？ 　　根据现行政策，一般诊疗费与门诊诊查费不可同时收费，具体执行哪一项价格项目由属地医保部门确定。如基层医疗机构收取一般诊疗费，即不可额外收取一般诊疗费中已包含的注射费、静脉输液费等费用。如基层医疗机构收取门诊诊查费，则可按实际提供医疗服务情况据实收取注射费、输液费等费用。 　　按照患者所需就诊情景，门诊诊查费可分为以下类别：一是门诊诊查费(普通门诊)，对应西医科室提供的建档、了解病情、阅读检查检验结果、制定诊疗方案等服务；二是门诊诊查费(中医辨证论治)，对应中医科室提供的建档、了解病情、辨证论治、作出证候诊断等服务；三是门诊诊查费(药学门诊)，对应提供的用药指导、咨询等服务；四是门诊诊查费(护理门诊)，对应提供的护理咨询、查体、评估等服务。针对复诊患者需要的开具药品、耗材、检查检验处方接续等服务，立项指南设立门诊诊查费(便民门诊)，方便复诊患者开药。此外，综合诊查类立项指南明确规定，副主任、主任医师(药师)提供的诊查服务，可在普通诊查费基础上进行加收。 【编辑:刘湃】|dfg|
